We accept a variety of health insurance plans and private pay options.  The Client Intake Process is provided through Office Ally and Practice Mate.  This computer based Electronic Medical Record (EMR) forum allow clients to complete and submit all required health and insurance forms.  This platform is use friendly and affords quick delivery of all required intake documents.  Please call or email if you would like more information regarding our intake procedures, specific services, and payment plans.
